Release Form Unlocking Together

Effective date: January 2023

DISCLAIMER: I hereby assume all risk during participation, and waive any and all rights, claims, demands, losses and damages which may be sustained and suffered on account of injury or damage to property, caused or alleged to be caused in whole or in part by the participation in Unlocking Together (referred to here also as ‘program’). I will indemnify and hold harmless, Madelyn Blair, and any of her affiliated organizations and governing bodies, officers, instructors, agents, and all representatives, together with all participants.

This liability waiver, release, and indemnification agreement applies to any form of participation related to participation in the Unlocking Together. This includes, but is not limited to, general participation, traveling to and from the program, and any and all activities related to Unlocking Together.

I consent to the use of any photographs, as well as any feedback or written comments by me in connection with this program for publicity, promotion, demonstration or other business purposes, in any medium, including the internet, and I waive any right to compensation in connection with such use.
